Pregnancy pillow and back pain: what it does
There is a second question in this subject that nobody answers, and it is the more useful one: why a pregnancy pillow sometimes leaves you stiffer in the morning. That is almost always a setting, and it is fixable in one night.
What the shape does to your hips
Lying on your side without support, the upper leg falls forward and down. The pelvis follows it and rotates, and the lower back has to absorb that rotation for however many hours you sleep.
A U-shaped pillow changes the geometry in two places at once. The front arm takes the upper knee and holds the thigh level, so the pelvis stays square. The back arm keeps you from rolling backwards, which is what undoes the position halfway through the night. Neither of those is a treatment — they are a shape holding a body in a particular arrangement.

Firmness decides whether it lasts
A pillow that holds the knee level in week one and flattens by week three puts you back exactly where you started, and it is easy to miss because the change is gradual. The test takes five seconds: press your palm into the middle of an arm — it should sink about an inch and a half and then push back.

Why does my pregnancy pillow hurt my back?

Three usual causes: your upper knee rests beside the front arm instead of on it, so the hips twist; you have added your normal head pillow on top of the curve, which pushes your neck forward; or the pillow sits too high up the bed, so the opening is above your thighs and the back arm holds nothing.
